Required Documents

Please present the following:

  • Your current U.S. passport, valid or expired
  • Completed passport application form DS-82 (Please note: If your previously issued passport was a 5 year validity passport you should use form DS-11; $135 fee applies)
  • 1 photo (2″ x 2″ inches or 5″ x 5″ cm) taken within the past 6 months (See our photo specifications page)
  • Fee of $110, payable in U.S. dollars or the Georgian Lari equivalent (accepted cash only)

Processing Time  

You should be prepared to allow from 10-14 business days to receive a new passport.

MINOR U.S. CITIZENS (under Age 16) must submit:

  • Form DS-11
  • Last U.S. passport
  • 1 photo (2″ x 2″ inches or 5″ x 5″ cm) taken within the past 6 months (See our photo specifications page)
  • Parents’ passports
  • Fee of $105, payable in U.S. dollars or the Georgian Lari equivalent (accepted cash only)
  • Progressive photos of minor’s development: Photos to demonstrate the child in the old passport is the same child presented at the time of interview.  Generally, one photo from each year since the prior passport was issued is sufficient.
  • Birth certificate or Consular Report of Birth Abroad (original for first time passport applicant; copy for passport renewal)
  • If the parents have authorized a legal guardian to apply for a passport on behalf of their child, the legal guardian must submit a passport or photo ID and a notarized power of attorney in English that specifically authorizes him/her to apply for the passport on behalf of the child.
  • If one or both parents are not physically present, then notarized forms DS-3053 should accompany the application. If the missing parent is not in the U.S. then the form should be notarized at the nearest U.S. Consulate.  A certified true copy of the missing parent’s passport or photo ID should accompany form DS-3053.
    If the written consent of the non-applying parent or guardian or sole custody cannot be obtained in a timely manner, then Form DS-5525(Statement of Exigent Special Family Circumstances) should be submitted with the minor’s passport application.

To report and replace your lost or stolen passport submit the following in person:

  • Completed and signed Form DS-64: Statement Regarding a Lost or Stolen Passport
  • Completed passport application form DS-11. DO NOT sign the form.                                                                                                If you cannot download the forms they will be provided when you come to the Consular Section.
  • It is important to report the loss or theft of a valid U.S. passport to the local police. Please bring your copy of the police report
  • Bring some form of government issued identification (e.g. driver’s license, residence card) as well as documentation that establishes your citizenship (e.g. birth or naturalization certificate, an expired passport).
  • 1 photo (2″x2″ inches or 5″x 5″cm) taken within the past 6 months (See our photo specifications page)
  • Fee of $135, payable in U.S. dollars or the Georgian Lari equivalent (accepted cash only)

You should be prepared to allow from 10-14 business days to receive a new passport. In cases where the applicant must travel urgently, the Consular section can issue an ’emergency passport’ which is usually valid for three months. This ’emergency passport’ can be exchanged later for a full validity passport at an embassy or consulate overseas or at a Passport Agency in the United States. Important Notice!

Passports reported lost or stolen at the Consular section are invalidated and can no longer be used for travel.
